Liverpool will receive the trophy at Anfield after their final home game of the season, against the side who helped them clinch the league – Chelsea.
The trophy and medals will be handed over during a special presentation on a purpose built platform on the famous Kop.
With fans unable to attend the event there are questions over a possible parade when safe to do so but there is more clarity regards the destination of commemorative medals for Jurgen Klopp's squad and his coaching staff.
Liverpool will receive 40 commemorative medals made of silver that are 2.25in (5.7cm) in diameter.
Premier League rules state they can be distributed to the manager, players and officials as the club see fit, as long as every player who has made at least five top-flight appearances this season gets one.
Clubs can request more medals if at least 39 players made five league appearances.
Which players will receive a medal?
There are 22 Liverpool players who are guaranteed to receive a commemorative Premier League medal.
Goalkeepers Alisson Becker and Adrian are both well over the required appearance total, so too defenders Virgil van Dijk, Trent Alexander-Arnold, Andy Robertson and Joe Gomez.
In midfield, Gini Wijnaldum, Jordan Henderson, Alex Oxlade-Chamberlain, James Milner, Fabinho, Adam Lallana and Naby Keita have sufficient outings, as do forwards Roberto Firmino, Mohamed Salah, Sadio Mane and Divock Origi.
Dejan Lovren, Joel Matip and Neco Williams will all be awarded a medal despite their combined Premier League appearances not approaching either of the other defensive back line.
And Xherdan Shaqiri qualifies with seven outings, while Takumi Minamino's recent substitute appearances against Crystal Palace and Arsenal moved him over the required five.
Who could also receive a medal?
Well, with the Reds just having games against Chelsea and Newcastle left to play until their season ends, one youngster has narrowly missed out on a medal.
Harvey Elliott still only has two appearances to his name meaning he cannot reach the amount needed. However, the teenager has already responded to criticism on social media with wisdom and maturity beyond his years.

Curtis Jones needs just one more appearance from the final two however to reach the magic marker of five. Jurgen Klopp said he will not hand out appearances like Christmas presents but maybe they'll be something in Curtis' stocking before the clash with Chelsea.
